Jefferson Center for Connected Care
The Jefferson Center for Connected Care develops and tests innovative approaches for a patient-responsive care delivery system. Our goal is to create replicable and sustainable models of care to reduce, and ultimately eliminate, health disparities.
The Jefferson Center for Connected Care operates under the leadership of the Executive Director Kristin Rising, MD MSHP, Professor and Director of Acute Care Transitions in the Department of Emergency Medicine in the Sidney Kimmel Medical College and Professor in the College of Nursing and College of Population Health, Thomas Jefferson University.
About the Center
The Jefferson Center for Connected Care serves as the hub for directed action initiatives that integrate research and pedagogy around the application of connected care within and beyond the health system to address health disparities. It will support the development and testing of innovative approaches for a patient-responsive care delivery system, with the ultimate goal of developing replicable and sustainable models to reduce and ultimately eliminate health outcome disparities.

Kristin Rising Receives Award from The MANNA Institute for Nutrition Research
Kristin Rising, M.D., M.S.H.P., Director of the Jefferson Center for Connected Care and Professor of Emergency Medicine received an $400,000 Award from The MANNA Institute for her project, “Nutrition Support Assessment Tool (NSAT) Refinement and Pilot Implementation Trial.”
